Clinical, or administrative, staff are then able to review an individual patient’s EVEREG status by locating the most recently submitted form within the patient’s forms list.

Instructions

Submitting an EVEREG status form

1. Log into the FPA dashboard as a Hospital Admin user or Professional user

2. Navigate to the Patients menu on the left side menu of the dashboard

   

3. Locate and click the patient entry you want to submit the EVEREG status form for.

4. Click on the Form Results menu on the left of the patient details area. Then click on the '+' button on the roght of the Form Results card.

5. Type EVEREG into the search box at the top of the modal that is displayed. Select the EVEREG form language version you want to use. Click the Add button. at the bottom right of the modal.

   

6. Fill out the form and click the Submit button at the bottom left of the form.

   

7. The submitted EVEREG tracking form is listed in the Forms Results forms list.

   

Locating an EVEREG status form for a specific patient

1. Log into the FPA dashboard as a Hospital Admin user or Professional user

2. Navigate to the Patients menu on the left side menu of the dashboard

3. Click the patient name whose EVEREG status you want to check.

4. Navigate to the Form Results menu on the left menu.

5. Enter EVEREG in the search field at the top of the Form Results card.

6. Click on the EVEREG form entry to display the form results you wish to view.

7. You can quickly navigate from one form entry to another using the next and previous buttons located in the top right of the form preview card.
