How to submit a form for a patient as a Clinician?

1 STEP 1: Go to the details page of a patient
2 STEP 2: Go to the ‘Results’ page
3 STEP 3: Select a form to submit on behalf of the patient
4 STEP 4: Submit the Form

STEP 1: Go to the details page of a patient

Go to Patients
Select a patient from the patients' list and click on a patient entry

STEP 2: Go to the ‘Results’ page

Depending on the type of form → Form, Questionnaire or Task, go to either Form Results, Questionnaire Results or Task Results

STEP 3: Select a form to submit on behalf of the patient

Click on the '+' sign in the top right corner

Select a form and click ‘Add’

STEP 4: Submit the Form

Fill out the form and click ‘Submit’
