The For Patient App (FPA) solution provides the ability to track the submission of patient data to the Spanish EVEREG hernia registry.

Tracking is managed by clinical, or administrative, staff via the dashboard by submitting the EVEREG form on behalf of the patient.

The submitted form is then stored as part of the patient's journey in an entry in the patient’s forms list.

Clinical, or administrative, staff are then able to review an individual patient’s EVEREG status by locating the most recently submitted form within the patient’s forms list.

Instructions

Submitting an EVEREG status form

  1. Log into the FPA dashboard as a Hospital Admin user or Professional user

  2. Navigate to the Patients menu on the left side menu of the dashboard

  3. Locate and click the patient entry you want to submit the EVEREG status form for.

  4. Click on the Form Results menu on the left of the patient details area. Then click on the '+' button on the roght of the Form Results card.

  5. Type EVEREG into the search box at the top of the modal that is displayed. Select the EVEREG form language version you want to use. Click the Add button. at the bottom right of the modal.

  6. Fill out the form and click the Submit button at the bottom left of the form.

  7. The submitted EVEREG tracking form is listed in the Forms Results forms list.

All submitted EVEREG forms are recorded with the date and time they are submitted. These submission details are listed in the forms list.
