The “Form Details” page displays all the previous entries for a particular form, with the option of creating a new form entry.

Overview

1. Form Title - The title of the form.

2. New entry - Tap on the button to be directed to the “create a new form entry” page.

3. Previous Entries - Tap on a list item to view the form responses for that date.

Completing Another Form Entry

To complete a form, tap on New Entry. You will need to answer all the mandatory questions. Once complete, tap the Submit button at the bottom of the page (you may need to scroll down to see the button).

Viewing a previous Entry

To view a previous entry, simply tap on the listing. You will be able to view all your previous form responses.

Please note: you must have an internet connection to access forms.
